W.e.f. 1^st October,2011 , it is mandatory to duly filled and signed of proposal form of oriental insurance by individual as well as his/her nominee to process his/her policy.

Collect a copy of your TDS CERTIFICATE for the year 2010 -11 from the Jalandhar Office . Certificate will be Issued only when you will submit a copy of your PAN CARD . so bring it along with you.
